Not Yet Receive My EA Form!

I suddenly remember something when I saw my husband EA Form. Why I have not received my EA Form until today? My company, boss, and most of the friends already received the Form but I have not. Is it possible that the Income Tax department has missed to send me the form? How could I submit the tax submission without the form? Oh my! I have to call them now and check what should I do now.
